The following information is necessary to enroll a student at Fairview. You do not have to fill out a Fairview Enrollment packet if your student is currently attending 8th grade in a Boulder Valley School Distict school. (click here for a Printable Version)

Please also provide a copy of your student’s:
- BIRTH CERTIFICATE
- IMMUNIZATIONS
- PROOF OF RESIDENCY-Original documents of a residential building contract, a deed of trust, a real estate buyer contract, a contract for lease/rent or most recent utility bill which has your name and address on it. A driver’s license cannot be accepted as proof of residency because the Motor Vehicle Division does not require a proof of residence.
- TRANSCRIPT-Only required if the student is transferring from a high school that is not in the Boulder Valley School District.
- GUARDIANSHIP- If you are not the birth parent, you must present an original copy of custodial papers or proof of legal guardianship.
- WITHDRAWAL FORM-Not required for students that are new to Fairview, starting in August, 2008. Required for students that are transferring from another high school for the current semester (January through May, 2008). Student must provide a signed withdrawal form with withdrawal grades from their previous school of attendance.

Enrollment packets are also available to be picked up in the Main Office and the Counseling Office.

Return the completed Enrollment Packets to the Registrar (in the Counseling Office) as soon as possible.
