Protocol for Requesting a Letter of Recommendation from your Counselor

A month prior to the letter due date, and no later than three weeks prior to the letter due date, let your counselor know that you will need a letter of recommendation for your college admission or for a scholarship application.  Please submit the following to your counselor:

  1. The pre-addressed envelope supplied by the college or a 9x12 inch manila envelope (preferably the type with the pull-strip adhesive) with:

·           The college or scholarship’s full and complete address

·           FHS’ return address in the upper left-hand corner utilizing this format:

·           At least two first-class stamps in the upper right-hand corner.

  1. $2.00 printing/handling/processing fee for each and every transcript.  Cash or checks are accepted.  The check should be made payable to Fairview High School.
  2. Any forms which are supplied by the college or scholarship to be completed by your counselor.  These forms are often referred to as “Secondary School Report (SSR)” and usually have sections that must first be completed by you (Name, Address, Social Security Number, Signature, even Bar Code Labels sometimes). An incomplete SSR may delay the mailing of your application or the college’s decision regarding your acceptance.  Therefore, it is very important that you review your SSR or equivalent form prior to giving it to your counselor.
  3. A detailed response to the Student Self-Assessment*. You are encouraged to type your responses. This form is available in the Counseling Office or online.
  4. A completed Activities Form* or resume of all in-school and outside-of-school activities.
  5. A competed Parent Brag Sheet*.
  6. Please place all of the forms in a folder with the due date for each on the outside of the folder.  It is most helpful if the deadlines are listed in chronological order, not alpha order. 

 

The counselor will mail your application (unless you applied on line), the SSR or any other forms, their letter and transcript all together in the envelope you have provided.

 

School policy prohibits the counselor from giving you the letter, even in a sealed envelope.  The colleges and scholarships want to insure that what the teacher is writing is a candid and forthright appraisal of your ability, thus this letter must be confidential.
