Important Insurance Note  
NOTE: All groups must submit proof of liability insurance at the same time the Facility Use request form is submitted. Liability insurance may be obtained as a separate policy or as a rider on homeowners insurance. Boulder Valley Schools must be listed as Additionally Insured on the certificate. If you are part of an organized group, the limits of liability are $1,000,000 per occurrence, $2,000,000 aggregate. If you are an individual, the limits of liability are $500,000 per occurrence, $1,000,000 aggregate. If you have any further questions, please feel free to call Stephanie Feind at 720-561-3765.

Facility Use Forms  
The links to your left allow you to fill out a facilities form within your browser window, then print it out and submit it to the Facility Use Program via mail, fax, or in person. The forms are in the web-standard PDF format. In order to view or print such documents, you must have the free Acrobat Reader loaded on your computer.

Now Hiring Building Monitors!  
Applications now being accepted for building monitors on a part-time, as-needed basis, weekends, and evenings; $10.00/hour. Work at locations throughout Boulder Valley School District including Boulder, Louisville, Lafayette and Broomfield. Must be dependable and security-conscious.
